Chronic Disease Management Program (CDMP)

HSS’ Chronic Disease Management Program (CDMP) is an innovative solution aimed at preventing the exacerbation of chronic disease through the utilisation of evidence based methodologies which target individual risk factors, and empower clients with the knowledge and skills to self manage their condition.

The CDMP is primarily managed telephonically, with in home visits organised as required. The program is delivered over approximately a six month period by HSS’ expert team of qualified healthcare professionals in collaboration with the client’s medical officer. HSS’ CDMP can be adapted to any chronic disease with modifiable risk factors and currently targets diseases including:

• Diabetes
• Cardiac
• Musculoskeletal
• Respiratory Conditions
• Mental Health
• Obesity

The CDMP is individually tailored to best meet the needs of the participating client. The services provided in the program include, but are not limited to:
• Health coaching
• Individual goal setting
• Behaviour & lifestyle modification
• In-home care as required
• Risk factor modification
• Education & support
• GP / Specialist engagement
