Senior Australians with complex care needs may be able to access funding for care services to be delivered in their homes, through government funded Home Care Packages (HCP).
To access Government subsidised aged care services, you will need to be registered with My Aged Care and undergo an assessment to find out if you are eligible for subsidised services. You may register and apply for an assessment by contacting My Aged Care on 1800 200 422 or through an online application.
Once assessed, Amplar Health may then guide you through the process and provide information on fees and costs associated with each program or service.
