Home Support Services is a health care organisation providing holistic care coordination, community health services including Nursing, Occupational/Physiotherapy & Personal Care to clients in their own home. We are a leader in community health care and our staff provides exceptional levels of holistic services in our client’s own home.

Registered Nurses

Home Support Services (HSS) is growing! We are expanding our national team of Casual Registered Nurses, with the flexibility to plan your own rounds, you will work autonomously with the constant support of an expert clinical team. We have opportunities available in Adelaide, Brisbane and Melbourne.

More about the role

  • Hands-on clinical support to clients in their own homes
  • Holistic care including comprehensive clinical assessments
  • Delivery of a range of clinical service including complex wound care, IV Antibiotic Therapy, and palliative care

Skills and Experience 

  • Current AHPRA Registration
  • Recent acute nursing experience
  • Wound Care Management
  • Continence nursing experience desirable
  • PICC Care/Dressing/Removal
  • Current National Police Clearance (or be willing to obtain one)
  • Current Drivers’ Licence

Intake Registered Nurse

HSS are seeking a skilled and passionate nurse to join our Registered Nursing team. As an Intake RN, you will be responsible for ensuring consumers receive paramount care and outstanding customer service.

More about the role

  • Provide rapid response to incoming member inquiries
  • Rapid triage of incoming inquiries from multiple sources and referrals
  • Providing clinical engagement to promote Rehab at Home program to potential customers
  • Liaising with key stakeholders and hospital liaison officers to engage referrals

Skills and Experience 

  • 2 years of nursing experience
  • Experience with private health Insurance funding models (desirable)
  • Current AHPRA Registration
  • Current Working With Children Check (or be willing to obtain one)
  • Current drivers’ license

Occupational Therapists

At HSS, we are growing and as a result, we are expanding our team of Occupational Therapists!  As an Occupational Therapist at HSS, you will use your clinical skills to coordinate all aspects of patient care from assessment and care planning to ongoing service delivery. This role is full time and based in Adelaide.

Personal Carers

At HSS, we are expanding our team of Casual Personal Carers in Adelaide to provide client-centered care for patients in their own homes.

Personal Carer duties involve; 

  • Provide care and support to patients
  • Meal preparation
  • Shopping
  • Transportation to appointments
  • Personal care and respite
  • Light domestic duties

Skills & Experience

  • Certificate III in Individual Support or equivalent, with either Cert 4 in Mental Health or Mental Health First Aid (Desirable)
  • Experience as a Personal Carer
  • Current driver’s license and willing to travel
  • Computer skills
  • Current Senior First Aid Certificate (or be willing to obtain one)
  • Current DCSI (or be willing to obtain one)

Clinical Nurse – Palliative Care Program

We are looking for an energetic and passionate Clinical Nurse to join our team in Melbourne. As a member of a multidisciplinary team, you will have the opportunity to assist in the provision of optimum palliative care services within the acute at home health care environment and subacute sites as required.

More about this role: 

  • Coordinating all aspects of patient care from assessment and care planning to providing direct patient care
  • Monitoring patient progression and modify the multidisciplinary, client-centered care plan
  • Liaising with hospitals, doctors and family members to provide safe palliative care to patients in their own home
  • Managing the logistics of Palliative home care services, including medication and stock requirements
