Registered Nurses

More about the role

  • Hands on clinical support to clients in their own home.
  • Holistic care including comprehensive clinical assessments
  • Delivery of a range of clinical service including complex wound care, IV Antibiotic Therapy and palliative care

Skills and Experience 

  • Current AHPRA Registration
  • Recent acute nursing experience
  • Wound Care Management
  • Continence nursing experience desirable
  • PICC Care/Dressing/Removal
  • Current National Police Clearance (or be willing to obtain one)
  • Current Drivers’ Licence

Registered Nurse Case Coordinator

The Opportunity

As HSS is growing. We are expanding our team of Registered Nurse Case Coordinators. As an RN Case Coordinator you will use your clinical skills to coordinate the acute care needs of HSS patients in a dynamic office based environment.

More about the role

This diverse and challenging role involves:

  • Facilitating a seamless transition of care from hospital to home
  • Coordinating all aspects of patient care from assessment and care planning to ongoing service delivery
  • Communicating with in-home service providers to monitor patient progression and modify the multidisciplinary, client centred care plan
  • Liaising with patients, families and medical teams to ensure collaborative care delivery
  • Managing the logistics of acute home care services, including medication and stock requirements
  • Coordinating a safe and effective discharge from HSS

Mental Health Case Coordinator

Duties and Responsibilities 

  • Use your mental health skills and experience to coordinate and deliver rapid response acute mental health services in the community, as an alternative to inpatient care.
  • Facilitate a seamless transition of care from hospital to the community.
  • Develop and implement holistic multidisciplinary patient centred care plans.
  • Collaborate with patients, families,  hospitals and our team of health professionals to provide  collaborative compassionate care to our clients within the community.
  • Deliver face to face community mental health services.

Skills and Experience 

  • Registered Nurse with Mental Health Qualifications
  • Current AHPRA Registration
  • Minimum 2 years mental health experience
  • Computer savvy
  • Exceptional communication and organisation skills
  • Hold a Current DSCI check  or be willing to obtain one and a Current National Police Clearance

Mental Health Case Coordinator

About us:

The Opportunity:

We are currently looking for a Registered Nurse – Mental Health Coach to join our health coaching wellness and prevention team, contacting, assessing & enrolling clients nationally

The successful applicant will be working within an interdisciplinary team of health clinicians including RN’s and Allied Health professionals. You will provide support primarily over the telephone to clients with a variety of mental health needs through a recovery-model lens.

Qualifications, Skills and Experience

  • Minimum 2 years experience in Mental Health as a Registered Nurse
  • Competent assessment skills
  • Experience in counseling and delivering therapy including group therapy.
  • Knowledge of a variety of therapeutic approaches, including:
    • Risk Assessment
    • Cognitive Behavioural Therapy
    • Dialectical Behavioural Therapy
    • Acceptance Commitment Therapy
  • Knowledge on the health care system and community supports relevant to mental health is desirable
  • Highly regarded if experience in specialty areas such as addiction or eating disorders
  • Excellent communication skills
  • Computer Savvy
  • Current DCSI  (or be willing to obtain one)
